Roles & Responsibilities
- Provide compassionate bedside care to residents, ensuring dignity and comfort
- Administer medications and treatments accurately and document per facility policy
- Monitor patient status, recognize changes and report them to the care team promptly
- Collaborate with nurses, physicians and allied staff to deliver coordinated care
- Educate residents and families about care plans and promote quality of life
- Maintain accurate, timely charting and comply with all regulatory standards
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of practice (Delaware)
- Current Basic Life Support (BLS) / CPR certification required or obtainable prior to start
- Previous LPN experience in long-term care or skilled nursing preferred
- Ability to pass facility background checks, drug screens and mandatory immunizations
- Strong communication skills and ability to collaborate with interdisciplinary teams
Education
- Graduation from an accredited LPN / vocational nursing program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Knowledge of long-term care protocols and documentation standards
- Ability to assess patient conditions and prioritize care needs
- Comfortable using electronic medical records and standard charting
- Strong interpersonal skills for working with residents and families
