Roles & Responsibilities

• Provide safe, compassionate bedside care and monitor patient status throughout the shift
• Administer medications and treatments accurately in accordance with facility protocols
• Document patient assessments, interventions and responses clearly and promptly
• Collaborate with RNs, physicians and allied staff to coordinate patient-centered care
• Support patient and family education, discharge planning and transitions of care
• Respond effectively to changes in patient condition and escalate when necessary
• Maintain a clean, safe environment and follow infection control and safety standards

Qualifications

• Active LPN license (practical/vocational nursing) — state licensure required
• Current BLS (Basic Life Support) certification; additional certifications a plus
• Prior experience in long-term care, rehab or acute care preferred
• Strong clinical assessment skills and excellent communication
• Ability to work independently during night shifts while collaborating with the care team

Education

• LPN: Vocational nursing diploma or certificate
• RN candidates: Associate Degree in Nursing or BSN (if applying as RN)

Knowledge & Skills

• Patient assessment and monitoring with attention to detail
• Medication administration and documentation best practices
• Infection control, wound care basics and safety procedures
• Effective communication with patients, families and interdisciplinary teams
• Time management and prioritization for busy night shifts
