Roles & Responsibilities

• Deliver compassionate direct patient care and prioritize patient comfort
• Administer medications and treatments safely and accurately
• Perform focused patient assessments and monitor vital signs
• Document care clearly and timely in the electronic health record
• Communicate changes in patient condition to the care team
• Educate patients and families about treatments and discharge plans
• Collaborate with nurses, therapists, and providers to support care goals

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania
• Graduation from an accredited practical/vocational nursing program
• Basic Life Support (BLS) / CPR certification (current)
• Previous clinical experience in long-term care or acute settings preferred
• Strong communication skills and ability to work well on a team

Education

• Vocational/Practical Nursing diploma or certificate (LPN program)

Knowledge & Skills

• Medication administration and safe documentation practices
• Wound care basics and routine nursing procedures
• Focused patient assessment and monitoring
• Proficiency with electronic health record documentation
• Time management, prioritization, and effective communication
