Roles & Responsibilities

• Provide direct, compassionate nursing care to residents as an LPN, focusing on safety and dignity
• Administer medications and treatments accurately and document in the medical record
• Perform routine assessments, monitor vital signs, and report changes promptly
• Collaborate with RNs, providers, and interdisciplinary team members to implement care plans
• Maintain clear, timely documentation to support continuity of care and regulatory compliance
• Educate residents and families about treatments, medications, and care plans
• Advocate for resident needs and contribute to a positive, patient-centered environment

Qualifications

• Current, active LPN license in Pennsylvania (or eligibility to obtain one quickly)
• Previous LPN experience in long-term care or skilled nursing preferred
• Basic Life Support (BLS) certification required or must obtain upon hire
• Comfortable administering medications and following established care plans
• Strong communication skills and ability to work well within a care team

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Thorough knowledge of practical nursing principles and long-term care practices
• Medication administration and safe documentation habits
• Resident assessment skills and identifying changes in condition
• Time management and prioritization in a busy clinical environment
• Team collaboration and clear communication with clinicians and families
