Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents
- Administer medications and treatments accurately and on time
- Monitor patient status and report changes to the interdisciplinary team
- Collaborate with nurses, therapists and other staff to develop and follow care plans
- Document clinical care and observations clearly in the patient record
- Assist with admissions, transfers and discharges to ensure smooth transitions
- Support residents' activities of daily living while promoting dignity and independence
Qualifications
- Current LPN license in Massachusetts (required)
- Basic Life Support (BLS) certification (preferred or obtain upon hire)
- Comfortable administering medications and performing routine nursing procedures
- Ability to pass background check and meet facility health requirements
- Previous long-term care or rehab experience preferred but not required
Education
- LPN diploma or vocational nursing certificate
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with charting and electronic health records
- Good communication and teamwork abilities
- Time management and organization in a fast-paced care setting
- Basic wound care and restorative nursing knowledge
