Roles & Responsibilities

• Provide compassionate bedside nursing care to residents, putting dignity and comfort first
• Administer medications and treatments accurately and on schedule
• Assess resident conditions and report changes promptly to the care team
• Document assessments, interventions, and care plans clearly in the medical record
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Support and mentor nursing assistants to promote a positive care environment

Qualifications

• Current Licensed Practical Nurse (LPN) credential; T-LPN candidates welcomed
• Active LPN license in the state of West Virginia or eligibility to obtain one
• Clinical experience in long-term care or skilled nursing is a plus
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Reliable, professional, and patient-centered with strong communication skills

Education

• Completion of an accredited Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Strong resident assessment and observation skills
• Comfortable with electronic medical record documentation
• Good teamwork and communication skills for interdisciplinary collaboration
• Ability to prioritize tasks in a fast-paced long-term care setting
