Roles & Responsibilities

• Provide compassionate direct patient care including vital signs, medication administration and basic treatments
• Collaborate with the interdisciplinary care team to implement individualized care plans
• Document patient status and interventions accurately in the electronic health record
• Assess patient needs and report changes promptly to the RN and provider team
• Support safe patient transfers, wound care basics and routine procedures per facility protocols
• Educate patients and families about plan of care and discharge instructions in a clear, respectful way

Qualifications

• Active LPN license in Pennsylvania (required)
• Current BLS (Basic Life Support) certification required
• Previous experience in long-term care, skilled nursing, or acute care preferred
• Strong interpersonal and communication skills; team-oriented and reliable
• Ability to pass background check and meet facility onboarding requirements

Education

• Completion of an accredited LPN/Vocational Nursing program; diploma or certificate

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and reporting of clinical changes
• Familiarity with electronic health records and accurate charting
• Infection control and standard precautions
• Time management and prioritization in a busy clinical environment
