Roles & Responsibilities

• Provide compassionate, competent nursing care to patients on your unit
• Administer medications and treatments accurately and on time
• Perform timely and thorough documentation of patient status and interventions
• Collaborate with RNs, physicians and interdisciplinary team members to coordinate care
• Monitor patients for changes and escalate concerns to appropriate providers
• Educate patients and families about care plans and discharge instructions
• Support safe patient transfers and maintain infection control standards

Qualifications

• Current LPN license (T-LPN accepted) as required by the assignment
• Valid basic life support (BLS) certification
• Previous LPN experience in long-term care or similar clinical setting preferred
• Reliable transportation as required by facility
• Ability to work evenings and weekends as specified in the contract

Education

• Graduate of an accredited practical nursing program; LPN diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with patient assessments and care planning
• Effective communication and teamwork on fast-paced units
• Basic wound care and vital signs monitoring
• Time management and prioritization for safe patient care
