Roles & Responsibilities

• Provide compassionate day-to-day nursing care for residents in a skilled nursing setting
• Administer medications and treatments accurately and document care thoroughly
• Assess resident condition, report changes promptly to the care team, and act on care plans
• Collaborate with RNs, therapists, and interdisciplinary staff to deliver coordinated care
• Support admission, discharge, and transfer processes with attention to detail
• Educate residents and families about care plans, medication, and safety measures
• Maintain a safe, clean, and respectful environment that promotes resident dignity

Qualifications

• Current LPN license (Licensed Practical Nurse) — Massachusetts licensure preferred
• Basic Life Support (BLS) certification (or willingness to obtain prior to start)
• Current immunizations and TB screening per facility requirements
• Ability to pass background check and drug screening as required by the facility
• Strong communication skills and a team-oriented approach to resident care

Education

• Completion of an accredited LPN/Vocational Nursing program and high school diploma or GED

Knowledge & Skills

• Solid knowledge of medication administration and infection control practices
• Comfortable with documentation in electronic medical records and paper charts
• Skilled at following care plans, clinical assessments, and reporting changes
• Able to prioritize tasks in a fast-paced long-term care environment
• Compassionate bedside manner and strong interpersonal skills with residents and families
