Roles & Responsibilities
- Provide compassionate day-to-day nursing care for residents in a skilled nursing setting
- Administer medications and treatments accurately and document care thoroughly
- Assess resident condition, report changes promptly to the care team, and act on care plans
- Collaborate with RNs, therapists, and interdisciplinary staff to deliver coordinated care
- Support admission, discharge, and transfer processes with attention to detail
- Educate residents and families about care plans, medication, and safety measures
- Maintain a safe, clean, and respectful environment that promotes resident dignity
Qualifications
- Current LPN license (Licensed Practical Nurse) — Massachusetts licensure preferred
- Basic Life Support (BLS) certification (or willingness to obtain prior to start)
- Current immunizations and TB screening per facility requirements
- Ability to pass background check and drug screening as required by the facility
- Strong communication skills and a team-oriented approach to resident care
Education
- Completion of an accredited LPN/Vocational Nursing program and high school diploma or GED
Knowledge & Skills
- Solid knowledge of medication administration and infection control practices
- Comfortable with documentation in electronic medical records and paper charts
- Skilled at following care plans, clinical assessments, and reporting changes
- Able to prioritize tasks in a fast-paced long-term care environment
- Compassionate bedside manner and strong interpersonal skills with residents and families
