Roles & Responsibilities
- Provide skilled, compassionate nursing care to residents following care plans and facility policies
- Administer medications and document accurately to ensure resident safety
- Assess resident conditions, report changes promptly, and collaborate with the interdisciplinary team
- Perform wound care, catheter care, and other skilled nursing procedures as required
- Educate residents and families about care plans and promote independence
- Maintain accurate, timely charting and support quality improvement initiatives
Qualifications
- Active LPN license (Licensed Practical Nurse) — required
- Current BLS (Basic Life Support) certification — preferred or willing to obtain
- Previous experience in long-term care, rehab, or skilled nursing — preferred
- Excellent communication skills and a resident-centered care approach
- Ability to pass background checks and meet facility onboarding requirements
Education
- LPN diploma or practical nursing program completion; High school diploma or equivalent
Knowledge & Skills
- Strong knowledge of medication administration and nursing documentation
- Familiarity with long-term care and rehabilitation nursing practices
- Skilled in resident assessment, infection control, and safety procedures
- Ability to work collaboratively with nurses, therapists, and support staff
- Good time management and prioritization in a busy clinical environment
