Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents according to plan of care
• Administer medications and treatments safely and document accurately in the chart
• Monitor resident status, recognize changes, and escalate to RN or provider as needed
• Perform focused assessments and update care plans to support positive outcomes
• Communicate clearly with residents, families, and the interdisciplinary team
• Support admission, transfer, and discharge processes with attention to detail
• Maintain a safe, clean environment and follow infection control practices

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in New Mexico
• Basic Life Support (BLS/CPR) certification required
• Minimum clinical experience in long-term care or skilled nursing preferred
• Ability to work overnight shifts and maintain clear communication with the care team
• Flexible, reliable, and professional — ready to step into a new team and make an impact

Education

• Graduate of an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Skilled in medication administration and documentation
• Solid patient assessment and vital signs monitoring skills
• Comfortable with wound care, ADL assistance, and common skilled nursing procedures
• Strong charting and electronic medical record (EMR) accuracy
• Good clinical judgment and the ability to escalate concerns appropriately
• Excellent communication and teamwork with interdisciplinary staff and families
