Roles & Responsibilities

• Provide high-quality, patient-centered nursing care in a rehab setting
• Perform assessments, develop care plans and monitor patient progress
• Administer medications and treatments safely and accurately
• Collaborate with therapists and the interdisciplinary team to support recovery goals
• Document care thoroughly and communicate changes in patient status
• Educate patients and families to promote independence and safe discharge

Qualifications

• Active Registered Nurse (RN) license; LPN considered depending on facility needs
• Experience or interest in rehabilitation nursing is a plus
• Current BLS certification or willingness to obtain prior to assignment
• Ability to pass facility onboarding requirements and background checks
• Strong communication skills and a team-first mindset

Education

• Graduate of an accredited nursing program (ADN/BSN for RNs or accredited LPN program)

Knowledge & Skills

• Patient assessment and clinical judgment in post-acute care
• Medication administration and IV or wound care basics common in rehab
• Familiarity with EMR documentation and care planning
• Ability to work autonomously during night shift and escalate concerns appropriately
• Team collaboration with PT/OT/Speech and case management
