Roles & Responsibilities

• Provide compassionate, bedside nursing care to residents as an LPN, making each day matter
• Administer medications and treatments accurately and on time
• Assess patients, report changes, and collaborate with the RN and interdisciplinary team
• Maintain clear, timely documentation in the medical record
• Assist with activities of daily living and promote resident dignity and independence
• Support a positive environment by communicating effectively with patients, families, and staff

Qualifications

• Current and active LPN license (state licensure required)
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Prior long-term care or rehab experience a plus but enthusiasm and strong clinical skills matter
• Reliable, professional, and able to pass facility-required background and health screenings

Education

• LPN diploma or equivalent practical nursing program completion

Knowledge & Skills

• Medication administration and documentation accuracy
• Clinical assessment and reporting to the care team
• Infection control and wound care fundamentals
• Strong communication and teamwork skills
• Time management and the ability to prioritize resident needs
