Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments as prescribed and document accurately
- Monitor and report changes in resident condition promptly to the care team
- Collaborate with nurses, therapists and physicians to implement individualized care plans
- Support residents with activities of daily living while promoting dignity and independence
- Participate in shift handoffs and maintain clear, thorough clinical documentation
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts required
- Current CPR/BLS certification (or willingness to obtain)
- Experience in long-term care or rehabilitative nursing is a strong plus
- Clear background checks and required health screenings (TB, immunizations) as per facility policy
- Strong communication skills and a patient-centered approach
Education
- Completion of an accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Knowledge of medication administration and charting best practices
- Familiarity with wound care, vital sign monitoring and infection control
- Ability to prioritize tasks in a fast-paced clinical environment
- Comfort working as part of an interdisciplinary rehab and nursing team
- Compassionate resident-centered care and good interpersonal skills
