Roles & Responsibilities

• Provide compassionate direct care to residents as an LPN, ensuring dignity and comfort
• Administer medications and treatments accurately and safely
• Assess resident status and communicate changes promptly to the care team
• Document care and observations clearly in the facility's medical records
• Collaborate with RNs, CNAs and interdisciplinary team members to create effective care plans
• Support resident and family education—help them understand care plans and next steps
• Participate in infection control, safety protocols, and quality improvement initiatives

Qualifications

• Active LPN (Licensed Practical Nurse) licensure — must be current and in good standing
• T-LPN or travel LPN experience preferred but enthusiastic new travel nurses are welcome
• Current Basic Life Support (BLS) certification preferred or willing to obtain
• Ability to work scheduled weekend shifts (Sun, Fri, Sat) and maintain reliable attendance
• Strong clinical judgment, charting skills, and ability to work as part of a team
• Up-to-date immunizations and ability to complete facility onboarding requirements

Education

• LPN diploma or equivalent vocational nursing program completion

Knowledge & Skills

• Medication administration and IV care fundamentals
• Resident assessment and effective documentation in electronic medical records
• Wound care and basic restorative nursing skills
• Excellent communication and teamwork with interdisciplinary staff
• Time management and prioritization in a fast-paced long-term care setting
• Understanding of infection prevention and patient safety protocols
