Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
• Administer medications and treatments accurately and document in the chart
• Perform thorough patient assessments and report changes to the care team
• Collaborate with nurses, providers and support staff to coordinate care plans
• Support residents with activities of daily living and promote independence
• Contribute to a positive unit culture by mentoring peers and communicating clearly

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
• Basic Life Support (BLS) / CPR certification required or willingness to obtain
• Comfortable with medication administration, documentation and resident assessments
• Reliable, professional and able to work the 2:45 PM - 11:15 PM shift
• Ability to pass required background checks and facility onboarding

Education

• Graduate of an accredited LPN program or accredited nursing program for RNs

Knowledge & Skills

• Strong patient assessment and clinical judgment skills
• Medication administration and documentation proficiency
• Familiarity with electronic medical records and charting practices
• Excellent communication and teamwork skills
• Ability to prioritize tasks in a fast-paced long-term care environment
• Compassionate bedside manner and resident-centered approach
