Roles & Responsibilities

• Provide skilled, compassionate bedside care as a licensed practical nurse
• Administer medications and treatments safely and accurately
• Perform routine assessments and document changes in patient condition
• Collaborate with RNs, providers and care teams to deliver coordinated care
• Maintain accurate electronic health record documentation and care plans
• Assist with admissions, discharges and safe patient transfers
• Adhere to infection control, safety protocols and facility policies to protect patients and staff

Qualifications

• Current LPN license in the state of Pennsylvania or compact license where applicable
• Basic Life Support (BLS) certification from an accredited organization (or willingness to obtain)
• Prior LPN experience in long-term care or similar setting preferred but new grads with strong clinical skills are welcome
• Up-to-date immunizations and negative TB screening per facility policy
• Ability to pass background check and drug screening
• Strong communication skills, dependability and a patient-centered attitude

Education

• Practical Nursing diploma or certificate
• Current LPN license

• Medication administration and basic wound care
• Vital signs monitoring and routine assessments
• Electronic health record (EHR) charting and documentation
• Infection prevention and safety practices
• Effective teamwork, time management and prioritization
• Compassionate patient and family communication
