Roles & Responsibilities

• Provide compassionate direct patient care that promotes comfort, dignity, and recovery
• Administer medications and treatments safely and accurately according to plan
• Monitor and document patient condition, reporting changes promptly to the care team
• Collaborate with nurses, therapists, and physicians to coordinate individualized care
• Support admissions, discharges, and transfers with professionalism and attention to detail
• Educate patients and families about care plans and self-care in a clear, empathetic way
• Maintain infection control and safety standards to protect patients and staff

Qualifications

• Active LPN license (Licensed Practical Nurse) — ready to practice confidently
• Current basic life support (BLS) certification or willingness to obtain upon hire
• Previous experience in long-term care, rehab, or skilled nursing is a strong plus
• Demonstrated ability to communicate effectively with patients, families, and the care team
• Reliable, accountable, and committed to delivering high-quality, patient-centered care

Education

• Completion of an accredited Practical Nursing program (LPN/LVN diploma or certificate)
• High school diploma or equivalent required

Knowledge & Skills

• Solid clinical assessment and medication administration skills
• Comfortable with electronic health records and accurate documentation
• Basic wound care, mobility assistance, and ADL support knowledge
• Strong interpersonal skills and teamwork mindset
• Ability to prioritize tasks in a fast-paced care environment
