Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document in patient records
• Collaborate with the care team to create and follow individualized care plans
• Monitor residents' conditions and report changes promptly to the healthcare team
• Support restorative and daily living activities to promote resident independence
• Maintain accurate and timely charting in the facility’s documentation system

Qualifications

• Active LPN (Licensed Practical Nurse) or T-LPN license (state license required)
• Current CPR/BLS certification or willingness to obtain prior to start
• Comfortable working 12+ hour rotating day shifts and weekends
• Strong communication skills and a team-oriented attitude
• Previous long-term care or skilled nursing experience preferred

Education

• Completion of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Medication administration and basic IV/IM familiarity where applicable
• Wound care, vital signs monitoring, and ADL assistance
• Charting and documentation practices, comfortable with electronic records
• Effective communication with residents, families, and interdisciplinary teams
• Time management and prioritization in a fast-paced care environment
• Infection control and resident safety best practices
