Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Autumn Lake Healthcare
• Administer medications and treatments accurately and on schedule
• Monitor and document patient vitals, changes, and responses to care
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Educate residents and families about care plans and medication instructions
• Maintain a safe, clean environment and follow infection control best practices
• Assist with admissions, discharges, and smooth transitions of care

Qualifications

• Current LPN license in West Virginia or ability to obtain state licensure
• Experience working as an LPN in long term care or skilled nursing settings preferred
• Current BLS certification from an accredited provider
• Ability to work 12-hour day shifts and meet the every-other-weekend requirement
• Strong communication skills and a team-first attitude

Education

• Diploma or certificate in Practical Nursing from an accredited program

Knowledge & Skills

• Practical knowledge of medication administration and documentation
• Familiarity with long term care workflows and resident-centered care
• Comfortable using electronic health records for charting
• Basic assessment skills and ability to escalate changes in condition
• Good time management and prioritization during busy 12-hour shifts
