Shift Type: Day shift, 7:00 AM - 3:30 PM; variable schedule to meet facility needs with weekends required as needed (No RTO) — guaranteed 40 hours/week

Roles & Responsibilities
- Provide compassionate, high-quality bedside care to residents in a long-term care setting
- Administer medications and treatments safely and accurately following facility protocols
- Perform thorough and timely documentation of resident assessments, interventions, and responses
- Collaborate with the interdisciplinary care team to develop and implement individualized care plans
- Monitor resident condition and report changes promptly to nursing leadership and providers
- Support residents with activities of daily living while promoting dignity and independence
- Participate in facility initiatives to improve patient safety and quality outcomes
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the practicing state or eligible for travel licensure
- Basic Life Support (BLS) certification required or must obtain upon hire
- Current immunizations and ability to pass TB screening and background checks
- Experience in long-term care or skilled nursing preferred
- Comfortable working weekends as needed and adhering to a strict start date
Education
- Graduate of an accredited Practical Nursing program; LPN diploma or certificate required
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Clear, compassionate communication with residents, families, and team members
- Proficient in electronic charting and timely documentation
- Infection control best practices and safe patient handling
- Ability to prioritize tasks in a fast-paced care environment
- Team-oriented mindset with a focus on patient-centered care
