Roles & Responsibilities

• Provide compassionate, high-quality bedside care to residents in a long-term care setting
• Administer medications and treatments safely and accurately following facility protocols
• Perform thorough and timely documentation of resident assessments, interventions, and responses
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Monitor resident condition and report changes promptly to nursing leadership and providers
• Support residents with activities of daily living while promoting dignity and independence
• Participate in facility initiatives to improve patient safety and quality outcomes

Qualifications

• Valid Licensed Practical Nurse (LPN) license in the practicing state or eligible for travel licensure
• Basic Life Support (BLS) certification required or must obtain upon hire
• Current immunizations and ability to pass TB screening and background checks
• Experience in long-term care or skilled nursing preferred
• Comfortable working weekends as needed and adhering to a strict start date

Education

• Graduate of an accredited Practical Nursing program; LPN diploma or certificate required

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Clear, compassionate communication with residents, families, and team members
• Proficient in electronic charting and timely documentation
• Infection control best practices and safe patient handling
• Ability to prioritize tasks in a fast-paced care environment
• Team-oriented mindset with a focus on patient-centered care
