Roles & Responsibilities
- Provide high-quality, compassionate patient care as a Licensed Practical Nurse
- Administer medications and treatments accurately and according to plan
- Assess patient condition and report changes to the care team promptly
- Document care thoroughly and maintain clear, timely charting
- Collaborate with interdisciplinary team members to support patient goals
- Support residents with activities of daily living and comfort measures
- Promote a safe, respectful and healing environment for patients and families
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of practice or ability to obtain one quickly
- Travel LPN experience preferred
- Current BLS/CPR certification or willingness to obtain before start
- Up-to-date immunizations and TB screening as required by facility
- Proven ability to work independently and as part of a team in a fast paced environment
Education
- Graduate of an accredited LPN or practical nursing program; diploma or certificate
Knowledge & Skills
- Strong medication administration and documentation skills
- Comfortable with electronic medical records and charting
- Excellent communication and patient education abilities
- Skilled in basic assessment, wound care and ADL assistance
- Good time management, prioritization and teamwork
