Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to adults in a long-term/skilled care setting
• Perform accurate assessments, document changes, and escalate concerns promptly
• Administer medications and treatments safely according to orders and facility policy
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Support residents with activities of daily living, mobility, and personal care needs
• Maintain clear, timely charting in the electronic health record and communicate with families

Qualifications

• Active Registered Nurse (RN) license — LPNs may also be considered depending on assignment
• BLS (Basic Life Support) certification required; ACLS or other certifications a plus
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical assessment, medication administration, and documentation skills
• Ability to pass facility background checks and meet onboarding requirements

Education

• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
• LPN: Diploma or certificate from an accredited practical nursing program
• High school diploma or GED required

Knowledge & Skills

• Skilled in patient assessment, wound care, and medication administration
• Comfortable with electronic charting and standard clinical documentation
• Strong communication skills for working with residents, families, and care teams
• Solid infection control practices and a focus on resident safety
• Ability to prioritize tasks and manage time effectively during busy shifts
