Roles & Responsibilities

• Provide patient-centered evaluation and evidence-based physical therapy treatment for inpatient/rehab patients
• Develop individualized plan of care and measurable goals in collaboration with patients and the care team
• Document skilled interventions, progress and discharge summaries clearly and timely
• Collaborate with nurses, physicians, and therapy colleagues to coordinate safe, efficient patient care
• Educate patients and families on mobility, fall prevention, home exercise programs and safe discharge plans
• Participate in team meetings, care planning and quality improvement initiatives to improve outcomes

Qualifications

• Current and unrestricted Physical Therapist license in Massachusetts (or ability to obtain prior to start)
• Doctor of Physical Therapy (DPT) preferred; Bachelor degree in a related field considered
• Minimum clinical experience working with adult rehab/long-term care populations preferred
• Basic Life Support (BLS) / CPR certification required or obtained upon hire
• Strong documentation skills and proficiency with electronic medical records
• Reliable, professional, and able to work collaboratively in a fast-paced rehab setting

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s degree in Physical Therapy or related field acceptable

Knowledge & Skills

• Skilled in therapeutic exercise, neuromuscular re-education and mobility training
• Knowledge of safe transfer techniques, gait training and assistive device prescription
• Familiarity with inpatient rehabilitation and restorative therapy models
• Strong clinical reasoning, assessment and goal-writing abilities
• Excellent communication skills with patients, families and interdisciplinary teams
• Time management and caseload prioritization to ensure quality patient care
