Roles & Responsibilities

• Provide compassionate direct care to residents at Andover Manor Rehab & Nursing, focusing on safety and dignity
• Administer medications and treatments accurately and document thoroughly
• Perform timely patient assessments and communicate changes in condition to the care team
• Support restorative and rehabilitation care plans to help residents regain independence
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain infection control standards and ensure a clean, safe environment
• Educate residents and families about care plans and encourage involvement in decision-making

Qualifications

• Active LPN (Licensed Practical Nurse) license — required
• Current BLS/CPR certification (or ability to obtain promptly)
• Comfortable working 12-hour day shifts and adapting to facility schedules
• Strong communication skills and a team-first attitude
• Previous experience in long-term care, rehab, or skilled nursing environments preferred

Education

• LPN diploma or certificate from an accredited nursing program

Knowledge & Skills

• Medication administration and accurate charting
• Basic nursing assessments and vital sign monitoring
• Wound care and pressure injury prevention practices
• Familiarity with electronic health records and documentation standards
• Infection prevention, safe lifting/transfers, and resident safety protocols
• Collaborative care planning and clear handoff communication
