Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans that promote recovery and independence
• Deliver therapeutic interventions and modalities with empathy and clinical precision
• Track patient progress, document outcomes, and communicate results to the care team
• Coach patients and families on safe mobility, home exercise programs and fall prevention
• Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes
• Maintain compliance with facility policies and state practice standards while advocating for patient needs

Qualifications

• Current and unrestricted Physical Therapist license in California or ability to obtain prior to start
• Doctor of Physical Therapy (DPT) preferred; related degrees considered
• Basic Life Support (BLS) certification required; other certifications a plus
• Strong clinical assessment and documentation skills with a patient-centered approach
• Prior experience in outpatient or post-acute settings preferred
• Excellent communication and teamwork skills; you treat colleagues and patients with respect

Education

• Doctor of Physical Therapy (DPT) preferred or Bachelor's/Master's in related field

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and functional mobility training
• Proficient with patient evaluation, goal setting and outcome measurement
• Comfortable with electronic medical record documentation and compliance protocols
• Ability to instruct patients and caregivers in safe transfer and home exercise programs
• Strong time management and multi-patient prioritization skills in a fast-paced setting
