Roles & Responsibilities
- Assess patients' functional abilities and design individualized treatment plans that help them regain independence
- Deliver evidence-based therapeutic interventions including therapeutic exercise and manual techniques
- Document patient progress clearly and timely in the medical record to ensure continuity of care
- Collaborate with interdisciplinary team members to coordinate comprehensive rehab goals
- Educate patients and families on home programs and safe functional independence strategies
- Monitor outcomes and adjust plans of care to maximize patient gains and satisfaction
- Maintain compliance with facility policies, safety standards, and regulatory requirements
Qualifications
- Current, unrestricted Physical Therapist license (or eligible) — Maine license preferred
- Active BLS/CPR certification
- Experience providing skilled physical therapy in post-acute or outpatient rehab settings preferred
- Strong documentation skills and comfort using electronic medical records
- Excellent communication and teamwork skills — you connect with patients and colleagues alike
Education
- Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy preferred)
Knowledge & Skills
- Skilled in evaluation and therapeutic exercise prescription
- Experience with mobility training, gait training, and fall prevention strategies
- Knowledge of rehab best practices and outcomes measurement
- Proficient clinical documentation and treatment planning
- Ability to educate patients and caregivers with clarity and empathy
