Roles & Responsibilities

• Evaluate patients’ physical function and design individualized treatment plans to maximize independence
• Deliver evidence-based therapeutic interventions and restorative exercises to improve mobility and quality of life
• Document patient progress clearly and timely in the medical record to support continuity of care
• Collaborate with nurses, physicians, and rehab team members to coordinate comprehensive care
• Educate patients and family members on home exercise programs, safe transfers, and fall prevention
• Monitor and adjust treatment plans based on patient response and goals to ensure meaningful outcomes
• Maintain equipment, safety standards, and professional practice in a fast-paced contract setting

Qualifications

• Current, unrestricted Physical Therapist license (NH licensure or eligibility) required
• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in related field considered
• BLS/CPR certification required; additional specialty certifications a plus
• Experience in outpatient, inpatient rehab, or skilled nursing settings preferred
• Comfort working a consistent day schedule including weekends during the 13-week assignment
• Ability to pass background checks, drug screening, and meet facility immunization requirements

Education

• Doctor of Physical Therapy (DPT) preferred; or Master’s/Bachelor’s in Physical Therapy or related field with appropriate licensure

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual therapy techniques
• Proficient with documentation and electronic medical records
• Strong clinical reasoning and goal-oriented treatment planning
• Excellent communication and teamwork skills when coordinating with interdisciplinary staff
• Safe patient handling and knowledge of assistive devices and adaptive equipment
