Roles & Responsibilities

• Provide high-quality, patient-centered physical therapy evaluations and treatments
• Design and implement individualized care plans that promote function, mobility, and independence
• Document patient progress thoroughly and communicate effectively with the interdisciplinary team
• Educate patients and families on home programs, safety, and prevention strategies
• Collaborate with nursing and rehab staff to ensure smooth transitions and excellent continuity of care
• Contribute to a positive, compassionate clinical culture that supports patient recovery

Qualifications

• Current, unrestricted Physical Therapy license in New Hampshire or compact state eligibility
• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy (preferred)
• Basic Life Support (BLS) certification required
• Experience in outpatient or contract rehab settings preferred but new grads with strong clinical skills encouraged to apply
• Up-to-date immunizations and ability to complete facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy acceptable

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, gait training, and functional mobility interventions
• Comfortable with clinical documentation and electronic medical records
• Strong clinical reasoning and patient education skills
• Excellent communication and teamwork abilities
• Ability to manage caseloads efficiently while maintaining compassionate care
