Roles & Responsibilities
- Evaluate patients’ functional status and develop individualized treatment plans
- Deliver evidence-based therapeutic interventions to improve mobility and independence
- Document assessments, progress notes, and discharge summaries accurately and timely
- Collaborate with nurses, physicians, and care teams to coordinate patient-centered care
- Educate patients and families on home programs and injury prevention strategies
- Monitor outcomes and adjust treatment plans to ensure measurable progress
Qualifications
- Active Physical Therapist license in New Mexico or eligibility to obtain one
- Current BLS/CPR certification
- Agencies must have a New Mexico caregivers account as noted
- Comfortable with documentation and common therapy software
- Strong communication skills and a team-oriented mindset
Education
- Doctor of Physical Therapy (DPT) or equivalent professional degree in Physical Therapy
Knowledge & Skills
- Musculoskeletal and neuromuscular assessment skills
- Skilled in therapeutic exercise, gait training, and functional mobility
- Patient education and home exercise program design
- Proficient clinical documentation and outcome tracking
- Ability to collaborate effectively across an interdisciplinary team
