Roles & Responsibilities

• Evaluate, plan and deliver individualized physical therapy care to promote patient function and independence
• Design and implement therapeutic exercise programs and mobility plans based on evidence and patient goals
• Document progress clearly and timely in the electronic medical record to ensure continuity of care
• Collaborate with nursing, OT, SLP and case management to develop comprehensive discharge plans
• Educate patients and families on safe mobility, home program exercises and fall prevention
• Monitor and adjust treatment plans based on outcomes, safety and patient tolerance
• Participate in team meetings and contribute to quality improvement and rehabilitation initiatives

Qualifications

• Current, active Physical Therapist license in Massachusetts or eligibility to obtain one promptly
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Current BLS/CPR certification
• Experience in post-acute, skilled nursing or rehab settings is a strong plus
• Excellent communication and teamwork skills; patient-centered care mindset
• Ability to complete clinical documentation and follow facility policies

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy accepted where applicable

Knowledge & Skills

• Skilled in mobility assessment, gait training and therapeutic exercise prescription
• Knowledge of prosthetic/orthotic considerations, transfer techniques and fall prevention
• Familiarity with modalities commonly used in rehab settings
• Strong clinical reasoning and outcome measurement use
• Comfortable using EMR systems and providing thorough documentation
• Ability to educate patients and families in an approachable, motivating way
