Roles & Responsibilities

• Evaluate and develop personalized rehabilitation plans to restore function and independence
• Deliver evidence-based therapeutic interventions, including manual therapy and therapeutic exercise
• Collaborate with nurses, physicians, and caregivers to ensure cohesive patient-centered care
• Document assessments, progress notes, and discharge plans accurately and timely
• Educate patients and families on home programs, fall prevention, and safe mobility
• Participate in team meetings and contribute to continuous quality improvement initiatives

Qualifications

• Valid Physical Therapist license for the state of Maine (or ability to obtain prior to start)
• Doctor of Physical Therapy (DPT) preferred; entry-level graduates considered
• Basic Life Support (BLS/CPR) certification required
• Comfortable documenting in electronic medical records and meeting regulatory standards
• Prior outpatient or contract experience preferred; strong assessment and manual skills
• Excellent communication skills and a collaborative approach to patient care

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in patient assessment, gait training, balance and mobility interventions
• Proficient with therapeutic exercise prescription and manual therapy techniques
• Knowledge of documentation best practices and Medicare/insurance guidelines
• Ability to prioritize caseloads and provide efficient, high-quality care
• Strong interpersonal skills to coach patients and work effectively with multidisciplinary teams
