Roles & Responsibilities

• Assess patients’ mobility, strength, balance and functional needs to create individualized care plans
• Deliver evidence-based therapeutic interventions to help patients regain independence
• Set clear, measurable goals and track progress through detailed documentation
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care
• Educate patients and families on home programs, safety strategies and fall prevention
• Maintain compliance with facility policies, safety standards and infection control procedures

Qualifications

• Current Physical Therapist license in New Hampshire, or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• BLS (Basic Life Support) certification required or willingness to obtain
• Prior experience in outpatient/contract settings preferred; strong assessment and manual therapy skills
• Comfortable working with diverse adults and collaborating across care teams

Education

• Doctor of Physical Therapy (DPT) from an accredited institution; state licensure required

Knowledge & Skills

• Skilled in therapeutic exercise, gait training and functional mobility interventions
• Proficient with documentation in electronic medical records and outcome measurement tools
• Strong clinical reasoning and the ability to prioritize caseloads
• Excellent communication skills with patients, families and staff
• Knowledge of safety standards, falls prevention and assistive device training
