Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at Rochester Manor
• Administer medications and treatments safely and according to facility protocols
• Perform thorough assessments and document changes in condition clearly and promptly
• Collaborate with the interdisciplinary team to develop and update care plans
• Support families with education, updates, and bedside comfort during care transitions
• Follow infection control, safety, and compliance standards to protect patients and staff
• Mentor and support CNAs and less experienced staff to promote high-quality care

Qualifications

• Active LPN license in the state of practice or eligibility to obtain New Hampshire licensure
• Current BLS/CPR certification required
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Solid medication administration and documentation skills
• Skilled in basic assessments, wound care, and ADL support
• Comfortable using electronic medical records and documenting accurately
• Excellent time management and prioritization during busy shifts
• Empathy, strong patient advocacy, and the ability to teach families
