Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN
• Administer medications and treatments accurately and document thoroughly
• Perform focused assessments and report changes in condition promptly
• Collaborate with RNs, therapists, and support staff to deliver coordinated care
• Assist with admissions, transfers, and discharge planning to ensure smooth transitions
• Maintain accurate charting and follow facility policies and infection-control protocols
• Support a positive resident experience through respectful communication and advocacy

Qualifications

• Active Licensed Practical Nurse (LPN) license required (WV licensure expected)
• Current CPR/BLS certification preferred or obtainable upon hire
• Previous experience in long-term care, skilled nursing, or similar settings helpful
• Strong communication skills and ability to work effectively on night shift
• Ability to pass facility-required background checks and health screenings

Education

• Graduate of an accredited practical nursing program (LPN diploma/certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic assessment skills and recognition of acute changes in patient condition
• Wound care, ADL assistance, and restorative care familiarity
• Infection prevention, safety, and compliance with facility protocols
• Electronic medical record charting and clear clinical communication
