Shift Type: Monday-Friday, 3:00 PM - 11:00 PM (Evening / 2nd shift)
Hours per Week: 40
Candidate Pay Rate: $27 per hour

Roles & Responsibilities
- Provide compassionate hands-on care to residents including activities of daily living
- Monitor and document vital signs and resident condition changes accurately
- Assist nursing team with safe transfers, mobility support, and positioning
- Support personalized care plans and communicate changes to the care team
- Maintain a clean, safe environment and follow infection control practices
- Engage with residents to promote dignity, independence, and quality of life
Qualifications
- Valid LNA / Licensed Nursing Assistant credential required
- T-LNA credential listed as a recognized title for this role
- Current CPR/BLS certification preferred or willingness to obtain
- Ability to pass background check and required health screenings
- Comfortable providing direct resident care in a long-term/rehab setting
- Strong communication skills and a team-first attitude
Education
- Completion of an approved Nursing Assistant / LNA program or equivalent
- High school diploma or GED preferred
Knowledge & Skills
- Skilled in basic nursing care and ADLs support
- Proficient in taking and recording vital signs
- Good documentation and observational skills
- Ability to follow care plans and escalate concerns appropriately
- Empathy, patience, and strong interpersonal skills with residents and families
