Roles & Responsibilities

• Assess patients' functional status and develop individualized, evidence-based treatment plans
• Deliver hands-on therapeutic interventions and progressive exercise programs to restore mobility
• Monitor and document patient progress in the medical record clearly and timely
• Coordinate care with nursing, physicians, and rehabilitation team members to optimize outcomes
• Educate patients and caregivers on home programs, safety, and fall prevention
• Adjust treatment based on patient response and best-practice clinical guidelines
• Promote a patient-centered environment that supports dignity, independence, and recovery

Qualifications

• Valid Physical Therapist license in Washington (or eligible to obtain WA license prior to start)
• Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Previous experience in outpatient, home health, or post-acute care settings preferred
• Strong clinical reasoning, documentation skills, and ability to work collaboratively

Education

• Doctor of Physical Therapy (DPT) preferred; Masters or Bachelor's in Physical Therapy accepted with appropriate licensure

Knowledge & Skills

• Skilled in patient assessment, gait training, balance and mobility interventions
• Familiarity with therapeutic modalities and progressive exercise prescription
• Comfortable using electronic health records and meeting documentation standards
• Excellent communication skills for patient education and team collaboration
• Ability to prioritize caseload and manage time effectively in a fast-paced setting
