Roles & Responsibilities
- Provide skilled nursing care to residents in a skilled nursing facility, delivering compassionate, individualized attention
- Administer medications and treatments safely and accurately
- Monitor patient condition and document assessments, interventions, and outcomes clearly
- Collaborate with the interdisciplinary team to coordinate care plans and support smooth transitions
- Educate residents and families on care plans, medications, and discharge instructions
- Maintain infection control and safety standards to protect residents and staff
- Support admissions, transfers, and discharge processes with professionalism and empathy
- Mentor and support bedside staff to promote high-quality resident care
Qualifications
- Active Registered Nurse or Licensed Practical Nurse license (RN/LPN) in good standing
- Comfortable working the 2:00 PM - 10:30 PM evening shift
- Demonstrated experience in skilled nursing or long-term care settings preferred
- Current Basic Life Support (BLS) certification preferred
- Clear background check and ability to meet facility onboarding requirements
Education
- Graduate of an accredited nursing program (RN: ADN/BSN or diploma; LPN: approved practical nursing program)
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Medication administration and IV/line familiarity as applicable
- Excellent documentation and charting habits
- Compassionate resident-centered care and strong communication skills
- Ability to work collaboratively with nursing and allied health teams
- Basic infection control and safety best practices
