Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on and evidence-based therapeutic interventions to restore function
• Document patient progress clearly and timely in the medical record
• Collaborate with nursing, rehab aides, and interdisciplinary teams to coordinate care
• Educate patients and families on home exercise programs and safe mobility strategies
• Participate in in-service training and contribute to quality improvement initiatives

Qualifications

• Current state Physical Therapist license (NH) or ability to obtain licensure prior to start
• Doctor of Physical Therapy (DPT) preferred; other PT degrees considered with valid licensure
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Comfort working independently in a contract/traveler role and adapting to facility processes
• Current immunizations and ability to pass background and drug screening as required

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable with proper licensure

Knowledge & Skills

• Strong assessment and clinical reasoning skills for adult and geriatric populations
• Skilled in therapeutic exercise, manual therapy, gait training, and assistive device fitting
• Proficient documentation and time-management to meet productivity and quality standards
• Excellent communication and teamwork to collaborate with interdisciplinary staff
• Ability to educate patients and caregivers in a clear, compassionate manner
