Roles & Responsibilities

• Provide compassionate, direct patient care that promotes dignity and comfort
• Administer medications and treatments accurately and safely
• Assess patients and promptly report changes in condition to the care team
• Document patient status, interventions, and outcomes clearly and timely
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Support admission, discharge, and transition planning to ensure smooth patient flow
• Maintain infection control and safety standards to protect patients and colleagues

Qualifications

• Current Licensed Practical Nurse (LPN) certification — ready to practice
• Recent hands-on LPN experience in long-term care, skilled nursing, or similar setting preferred
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Up-to-date immunizations and ability to complete facility onboarding requirements
• Ability to pass background check and drug screening

Education

• LPN diploma or vocational nursing certificate

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with charting and electronic health record documentation
• Effective communication with patients, families, and interdisciplinary teams
• Time management and prioritization in a fast-paced care environment
• Familiarity with infection control practices and patient safety protocols
