Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long term care setting
• Administer medications safely and accurately following facility protocols
• Perform thorough patient assessments and monitor changes in condition
• Document care and observations clearly in the electronic health record
• Collaborate with nurses, therapists, and care team members to support patient-centered plans
• Assist with admissions, transfers, and discharge activities to ensure continuity of care
• Follow infection control and safety procedures to protect patients and staff

Qualifications

• Active LPN license in Maryland (must be in good standing)
• Current BLS/CPR certification or willingness to obtain on start
• Previous LPN experience in long term care, skilled nursing, or a related clinical setting preferred
• Strong communication skills and a patient-first attitude
• Ability to pass required background checks and drug screening

Education

• Practical Nursing diploma or certificate
• High school diploma or GED

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable taking and interpreting vital signs and patient assessments
• Familiar with charting in electronic health records and accurate documentation
• Knowledge of wound care basics, infection control, and safety protocols
• Excellent time management and ability to prioritize in a busy clinical environment
• Team player with strong interpersonal and patient education skills
