Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the facility
• Administer medications and treatments accurately and safely according to orders
• Monitor and document vital signs, patient condition, and response to interventions
• Collaborate with nurses, therapists, and physicians to develop and follow care plans
• Educate patients and families about care plans, medications, and follow-up
• Maintain clear, timely documentation in the medical record
• Adhere to infection control, safety protocols, and facility policies to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license for Pennsylvania required
• Current BLS/CPR certification required; additional certifications a plus
• Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
• Ability to pass background check, drug screen, and required health screenings
• Strong communication skills and ability to work effectively on a collaborative team

Education

• Graduate of an accredited Practical Nursing program; LPN diploma or certificate required
• High school diploma or GED required

Knowledge & Skills

• Hands-on patient care and medication administration
• Vital signs assessment and basic physical assessment skills
• Charting and documentation in electronic or paper medical records
• Time management, organization, and prioritization in a busy clinical setting
• Compassionate bedside manner and clear communication with patients and families
