Roles & Responsibilities

• Administer medications and treatments safely and accurately
• Monitor and document vital signs and patient status with attention to detail
• Provide hands-on patient care and assist with daily living needs
• Collaborate with the interdisciplinary team to support individualized care plans
• Communicate clearly and compassionately with patients and families
• Perform timely, accurate charting in the electronic medical record
• Support infection control practices and maintain a safe environment

Qualifications

• Active Licensed Practical Nurse (LPN) license (must be valid in New Mexico)
• Willingness to work night shifts and adapt to facility scheduling
• Ability to pass background check, drug screen, and facility credentialing
• Customer-focused attitude and professional communication skills
• Previous LPN experience in long-term care or rehab settings is a plus

Education

• Practical Nursing diploma or certificate (LPN)

Knowledge & Skills

• Medication administration and basic IV/PO knowledge
• Patient assessment and documentation best practices
• Basic wound care and ADL assistance
• Familiarity with electronic medical records and charting
• Strong teamwork, time management, and patient-centered care
