Roles & Responsibilities

• Assist patients with activities of daily living (bathing, dressing, toileting) with dignity and respect
• Support mobility and safe transfers, reducing fall risk
• Monitor and report changes in patient condition to nursing staff promptly
• Help create a comfortable, home-like environment for residents
• Document care accurately and timely in the patient record
• Collaborate with the interdisciplinary team to support rehabilitation goals
• Provide compassionate companionship and engage residents in meaningful activity

Qualifications

• GNA certification (required)
• Current CPR/BLS certification (or willing to obtain)
• Up-to-date immunizations and TB screening per facility policy
• Ability to pass background check and required health screenings
• Previous experience in long-term care, rehab, or geriatrics preferred
• Strong communication skills and a team-oriented attitude

Education

• High school diploma or equivalent; GNA certification required

Knowledge & Skills

• Comfortable assisting with ADLs and basic nursing tasks
• Knowledge of infection control and standard precautions
• Able to follow care plans and report observations clearly
• Good time management and the ability to prioritize resident needs
• Empathy and patience working with older adults and those in rehab
