Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing setting
- Administer medications and treatments accurately and on schedule
- Perform routine assessments and promptly communicate changes in condition
- Document care thoroughly and maintain accurate medical records
- Collaborate with the interdisciplinary team to develop and update care plans
- Support admissions, discharges, and transitions with professionalism and empathy
- Maintain infection control and safety standards to protect residents and staff
- Mentor and support nursing assistants and other team members when needed
Qualifications
- Active Licensed Practical Nurse (LPN) license (Massachusetts preferred/required)
- Current BLS/CPR certification preferred
- Comfortable working 12-hour day shifts in a skilled nursing/rehab environment
- Strong communication skills and a patient-centered mindset
- Previous experience in long-term care or skilled nursing is a plus
Education
- Graduate of an accredited Practical Nurse (LPN/LVN) program
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and catheter/IV familiarity relevant to LPN scope
- Resident assessment and care planning fundamentals
- Electronic documentation and charting best practices
- Effective communication with residents, families, and the care team
- Time management and prioritization during busy 12-hour shifts
