Roles & Responsibilities
- Provide compassionate, resident-centered nursing care as an LPN
- Administer medications and treatments safely and document accurately
- Monitor and record vital signs, reporting changes to the care team
- Assist residents with activities of daily living while preserving dignity
- Support care planning by communicating observations to nurses and clinicians
- Maintain clear, timely documentation in resident charts
- Follow facility policies and infection control practices to keep everyone safe
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Current BLS/CPR certification or ability to obtain promptly
- Ability to pass background check and drug screening
- Strong communication and teamwork skills; dependable and patient-focused
- Previous LPN experience in long-term care or similar settings preferred
Education
- Completion of an accredited practical nursing program (LPN diploma or certificate)
- High school diploma or GED
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care and monitoring for changes in condition
- Comfortable with charting, electronic or paper-based medical records
- Skilled at resident assistance, transfers, and ADLs
- Good observational skills and the ability to escalate care concerns appropriately
