Roles & Responsibilities

• Provide compassionate, direct patient care as a Licensed Practical Nurse at the facility
• Administer medications and treatments accurately and document them in the patient record
• Perform thorough patient assessments and report changes promptly to the care team
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Maintain patient safety and infection control practices at all times
• Educate patients and families on care plans and discharge instructions with clarity and respect
• Accurately complete documentation and handoffs to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) credential (required)
• Valid state LPN license appropriate for practice in New Mexico
• Basic Life Support (BLS) or CPR certification preferred or required by facility
• Ability to pass background check and meet facility onboarding requirements
• Strong communication skills and a team-first attitude

Education

• LPN diploma or certificate from an accredited practical nursing program

Knowledge & Skills

• Medication administration and documentation best practices
• Clinical assessment and vital sign monitoring
• Charting in electronic medical records and accurate documentation
• Infection control and patient safety protocols
• Effective communication with patients, families, and interdisciplinary teams
