Roles & Responsibilities

• Provide compassionate direct patient care, keeping dignity and comfort front and center
• Administer medications and treatments accurately and on schedule
• Perform thorough assessments and report changes promptly to the care team
• Document care clearly and efficiently to support continuity and quality
• Partner with nurses, therapists, and leadership to create strong care plans
• Support patient and family education with clear, respectful communication

Qualifications

• Active LPN license in Pennsylvania required
• Current BLS/CPR certification required; other specialty certifications are a plus
• Previous LPN experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and a patient-first attitude
• Ability to pass required background checks and health screenings

Education

• LPN diploma or certificate from an accredited nursing program required
• High school diploma or GED required

Knowledge & Skills

• Solid medication administration and documentation skills
• Comfortable performing routine assessments and reporting findings
• Basic wound care and ADL assistance knowledge
• Team-oriented with good time management and prioritization
• Familiarity with electronic charting and clinical workflows preferred
