Roles & Responsibilities

• Provide compassionate bedside nursing care to residents at Bear Canyon Rehabilitation Center
• Administer medications and follow physician orders accurately and safely
• Perform focused patient assessments and report changes to the care team
• Document care and interventions clearly in the facility electronic health record
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Educate patients and families about treatment plans and safe transitions
• Assist with admission, transfer, and discharge processes to ensure continuity of care
• Maintain infection control standards and ensure a safe environment for patients

Qualifications

• Current Licensed Practical Nurse (LPN) credential in good standing
• Completion of an accredited practical/vocational nursing program
• Basic Life Support (BLS) / CPR certification required or obtained upon hire
• Previous LPN experience in long-term care, rehab, or acute care preferred
• Ability to pass background check and drug screening as required by facility
• Strong communication skills and a patient-centered approach

Education

• Graduate of an accredited LPN / Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Medication administration and basic pharmacology knowledge
• Skilled in vital signs, wound care basics, and ADL support
• Accurate charting and documentation practices, familiarity with EHR systems
• Infection control, safety protocols, and regulatory awareness
• Effective communication with patients, families, and interdisciplinary teams
• Time management and prioritization in a fast-paced clinical environment
