Roles & Responsibilities
- Provide compassionate direct patient care and daily living assistance that improves resident quality of life
- Administer medications and treatments accurately and document thoroughly
- Monitor patients’ health status and promptly report changes to the care team
- Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
- Maintain a clean, safe environment and follow facility infection control policies
- Communicate professionally and empathetically with residents and families to build trust
Qualifications
- Active LPN license in Massachusetts (or compact/reciprocal where applicable)
- Current BLS/CPR certification (or willingness to obtain promptly)
- Comfortable working 12-hour day shifts
- Strong interpersonal skills and a team-first attitude
- Prior experience in long-term care, rehab, or skilled nursing is a plus
- Up-to-date immunizations per facility policy
Education
- Graduation from an accredited practical nursing program (LPN/LVN certificate or diploma)
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and vital sign monitoring
- Familiarity with electronic health records and documentation best practices
- Ability to prioritize tasks in a fast-paced care environment
- Excellent communication and patient advocacy skills
- Compassionate bedside manner and strong observational skills
