Roles & Responsibilities

• Provide compassionate direct patient care and daily living assistance that improves resident quality of life
• Administer medications and treatments accurately and document thoroughly
• Monitor patients’ health status and promptly report changes to the care team
• Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
• Maintain a clean, safe environment and follow facility infection control policies
• Communicate professionally and empathetically with residents and families to build trust

Qualifications

• Active LPN license in Massachusetts (or compact/reciprocal where applicable)
• Current BLS/CPR certification (or willingness to obtain promptly)
• Comfortable working 12-hour day shifts
• Strong interpersonal skills and a team-first attitude
• Prior experience in long-term care, rehab, or skilled nursing is a plus
• Up-to-date immunizations per facility policy

Education

• Graduation from an accredited practical nursing program (LPN/LVN certificate or diploma)

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and vital sign monitoring
• Familiarity with electronic health records and documentation best practices
• Ability to prioritize tasks in a fast-paced care environment
• Excellent communication and patient advocacy skills
• Compassionate bedside manner and strong observational skills
